    Grade Changes: After submitting your final grade roster, all grade changes must be submitted via the Grade Change Form. This includes grade changes due to clerical errors or miscalculations. Grade changes are limited to one calendar year. For more information, click here

    The required form is for GRAD Program transfer and substitution documentation.  The form must be filled out and uploaded to the student file.

    Staff will need to provide the form to students when requested and assist them in filling it out. This form should also be used when international transfer credit is requested.

    **In order to have a Transfer Course Equivalency Change reviewed, the student MUST have official transcripts and an initial evaluation complete.

    Exception Form Instructions:

    1. Fill out the form in its entirety.
    2. Send the form to the Program Coordinator (PC) for approval.
    3. Send the finalized form with appropriate signatures to evaluators@park.edu to be processed.

    Submit this form if the student has:

    • Completed a minimum of 108 credit hours.
    • Does not have a previous bachelor degree awarded.
    • The evaluation team will consult with the Assistant Provost for Academic Success to ensure compliance with standards for liberal education, writing-intensive coursework, and degree-level qualifications.
    • Once the analysis is complete, staff will be informed of the decision, and all relevant documentation and exceptions will be updated accordingly.
    • All core course exceptions require review by the appropriate Academic Department. Please submit the Exception Form separately to initiate this process.
    • Students should enroll in ALL remaining classes that do not have a pre-requisite.
